Job Title: Intake Specialist
Reports To: Office Manager ~ Team Leader
Job Summary:
The Intake Specialist will coordinate all referrals, prepare admission charts, schedule necessary disciplines, and perform data entry of OASIS and Plan of Cares. Working collaboratively with the Director of Nursing the Intake Specialist assures exceptional service to clients, families, referral sources and co-workers.
Primary Duties & Responsibilities:
- Respond to incoming calls from referral sources and potential clients. Track all inquiries and referrals.
- Schedule all disciplines with the appropriate staff according to policy.
- Complete data input of all notes, OASIS & Plan of Care information according to policy.
- Determine coverage availability and insurance verification for requested services, and pass admission information to the Director of Nursing.
- Process information in the electronic data system and export according to policy.
- Troubleshoot complaints/issues and provide a satisfactory resolution to problems.
- Prepare admission charts. Review documentation for accuracy and completeness pertaining to client charts.
- Assures implementation of quality assurance program.
- Pre-process payroll according to policy.
- Verify funds daily with UGS and report to the finance department.
- Coordinate monthly on-call schedule according to policy
- Coordinate in-services and monthly case management meetings.
- Complete Medicare RAPS and final claims according to agency policy.
- Complete Epocrates and prepare educational material for clients.
- Coordinate all medical equipment for client care.
Other Duties & Responsibilities:
- Communicates both verbally and in writing with supervisors, physicians, field staff, and office personnel, clients’ and/or clients’ families.
- Ensure Agency is operating according to standards.
- Compose and type routine corespondence and memoranda using Microsoft Word.
- Ensure all weekly reports submitted timely and accurately.
- Complete required work on a daily basis.
- Answer telephones directing the caller to the appropriate individual, when needed.
- Performs all functions in a professional manner and collaborate with other health care team members.
- Ensure PTA notes are signed by a PT each week.
Knowledge, Skills and Abilities:
- Broad knowledge of healthcare regulations.
- Excellent computer skills including knowledge of Microsoft Office.
- Ability to build meaningful relationships with all staff, and other health providers.
- Ability to coordinate individuals into a smooth and healthy functioning team.
- Ability to communicate clearly and effectively verbally and in writing.
- Ability to work overtime, holidays, and weekends, if requested by Supervisor.
- Attention to detail with emphasis on accuracy and quality.
- Ability to prioritize work to balance multiple projects and deadlines while working independently.
- Ability to excel in a fast-paced work environment while being compassionate and confident.
Professional Qualification:
- One year related experience in professional environment
Requirements:
- Have a valid driver’s license, reliable vehicle and current auto insurance.
- Clear criminal record of theft and violence.
- High school diploma or equivalent.
- Current CPR and TB.
- Able to read, speak and write English fluently in all aspects required to fulfill job functions.
- Able to stand, bend, lift up to 35 lbs, walk, and sit in periods intermittently.
