Job Title: Physical Therapist
Reports To: Director of Nursing
Professional Qualification:
Graduate from an accredited Physical Therapy program. Possess a current license as a Registered Physical Therapist in the state of Michigan. Minimum work experience of three (3) years.
Position:
Responsible for assessing, planning, organizing, administering, documenting, coordinating and evaluating client care. Practices in accordance with the Michigan Public Health Code, and Seasons Change Home Health Care Policies and Procedures. Behavior reflects the agency’s Mission and Philosophy. Primary Duties:
- Performs the initial evaluation to determine the client’s level of function.
- Identifies the client’s problems and sets goals.
- Develops client care plan in consultation with the Physical Therapy Assistant and physician to achieve goals.
- Performs necessary treatment to achieve client’s goals.
- Follow all regulations set forth by the State of Michigan in regards to Physical Therapist guidelines.
Secondary Duties:
- Follows the agency policy and procedure manual.
- Friendly and courteous to all employees and clients.
- Positive community involvement.
- Assist and review evaluations with the field staff.
